Wrong Policy Name

Received a call from Fgro Red to market a shirt term policy,all negotiations were with me and risk assessment done on me,the debit order on my name as well. The policy was issued in my wife's name and nobody can tell me how it happened. My wife now needs to cancel the policy. I have never dealt with a situation like this.....please explain how the policy was issued in my wife's name when she NEVER spoke to you!!
