<p>My ****her passed away the 7 May 2017. My mother is still in hospital. After enquiring what forms/documents Fedhealth need for my mother to become the main member I send the following to ********** (provided by them) – my ID, BANK STATEMENTS, DEATH CERTIFICATE – so monthly payments can go off from my account in future – ***. I also send the CONTINUATION FORM & DEATH CERTIFICATE to ********** to change my mother as the main member – ***. The 30/05/2017 I again send the CONTINUATION FORM & DEATH CERTIFICATE to ********** as well as my ID, BANK STATEMENTS & DEATH CERTIFICATE to ********** The 31/05/2017 I send the DEATH CERTIFICATE, CONTINUATION FORM, my ID & BANK STATEMENTS to ********** with “Medical Aid: Fedhealth ********** 5857 [M ********** 738]” in the subject. The 1/06/2017 I send the following to ********** – letter for permission to recover monthly payments from my account, BANK LETTER, ID of myself & my mother & CONTINUATION FORM. I also asked them to send me the banking details, so I can deposit this month’s payment in the mean while. Earlier this afternoon, I received a phone call from Intercare Subacute Hospital Tygervalley that their confirmation clerk spoke to a Call Centre Agent, Anita at Fedhealth, who informed her that authorization was granted for my mother’s stay with them was up until 31 May 2017. According to Anita, as of 1 June 2017, my mother is no longer a valid member of Fedhealth. She also could not see any application for new membership for my mother. The reference number for this telephonic conversation is ********** 7. I also tried to resolve this problem through Fedhealth’s website, but correspondence with DARREN (Customer Service) was of no help.</p> <p> </p>
