Who is responsible?!

Dear FedHealth<br> I joined this medical scheme because of the female benefits that come with the Maxima ENtrySAver profile<br> I recently went to to the gynaecologist (27/07/2015) and was requested to pay R500 upfront fee to see the gyne, which I paid.<br> Two days ago, I received a text stating that an additional R854,70 was paid to the Gyne from my savings account. And the R500 i had paid would not be refunded due to an incorrect code.<br> This gyne consultation is covered under my Women's Health benefit so essentially no fees should be deducted from my savings. I did everything I was supposed to do
