Claim reversed without reason

Why has my Daughter (Dep01) paed consult claim been reversed without reason?

Service date 19.04.2017

I have terminated my membership due to your lack of communication yet you are still giving me such grief. I paid my monthly contributions on time every month without fail yet you decide to reverse an APPROVED claim without notifying me whatsoever.

About a week ago I requested an EFT form to complete for my Savings refund as my banking details have changed, I have to date not received a response, apart from the automated mail which confirms receipt of my email!!

I expect a speedy response!!
