Bad service. Now canceling my medical aid

I have so much issues with this medical aid I have requested for my vault to be activated its now two weeks I only received a email today advising they can't see the forms. Its unacceptable service none stop bills that is not paid I'm just to a point now. Please cancel my medical aid due to rude call center agents. When called in they give me the runaround.And always advising me that they did not receive my documents. I had it please cancel this medical aid. Who ever made changes to this medical aid you have just made something working in to worse. I hope the media can see what fedhealth is doing to people
