Wrong information and sad service

Good day Iam just upset how can I be listed as debt review when I never app**** for debt review, I even have a letter from the company which experian listed me on the credit report,, I submitted all my documents with the letter stating that I did not apply for debt review,,and lastly I still have old accounts thats closed for more than 5 years on my report,, sad service
