Wrong fuel dismissal

I'm not sure if I'm wrong or what but what happens when someone at engen as an employee makes a mistake of dispatching an incorrect fuel in a vehicle and gets dismissed , although there's insurance for that I would like to find out if it's engens policy to dismiss a first time attempt of wrong fuel dispatched in a vehicle? Or are there disciplinary procedures followed .
