Pothole damage claim - no response

<p>Please can I get a response to my email sent. I'm not sure if Simon Z is getting my emails. It's been two weeks. Please can a working document,i.e. a file that can open be used e.g. PDF. otherwise I am unable to submit (the word document I received does not open). Thanks.</p>
