Discovery life, no feedback after 3 weeks on my tax certificate

Had a retirement fund with them and to date, they have been unable to find/send me the required tax certificate for contributions. In fact! I have been passed around from department to department because they can find no record of me. Yet, payments were made.

It's been 3 weeks since I logged my query with no feedback. But you can bet that I have been called every week by their cold calling sales departments to sell me more products.
