Sent cancellation form but acc still not cancelled

I called in at discovery card in October 2013 and asked for a settlement and then i subsequently paid the settlement amount, they then requested that i send a cancellation form which i did on the 19th of December 2013 (still have the email), to my surprise i'm still receiving account statements informing me that i have a balance of R 200 +, can you please cancel this account.

It's been 2 years since i posted this review and still my credit card is not cancelled instead I'm being sent statements advising that i still owe discovery, please cancel this card once and for all.
