Unprofessional Conduct at DisChem

<p>In October 2016 I went to the clinic at Dischem Randpark ridge for a Vitamin B injection as I have for the last 10 years without any problems</p> <p>This time I encountered Michelle Labuscagne a very sullen, terse individual devoid of the normal courtesy of a health care worker</p> <p>One of the points was the door was not closed at start of consultation</p> <p>I signed the part of the form giving permission for the injection and absolving Dischem of any responsibility and crossing out the paragraph giving Dischem blanket consent to share any of my personal information with whoever they see fit</p> <p>Before removing my pants for the injection I went and closed the door myself and incurred a remark for preferring to sit down to minimize the possibility of a spasm</p> <p>I figured she was having a bad day so let it go</p> <p> </p> <p> On the 15th December 2nd time 2months later even worse </p> <p>Same person, same sullen attitude, same lack of courtesy </p> <p>For example no explanation offered for the inordinately long time on the computer.. eg: sorry computer is slow.. turns out part of the time was taken in printing the contentious form</p> <p>In view of the sometime long que of patients what about pre-printing ?</p> <p>Her verbal communication s****s consisted of “your ID number ?" </p> <p>Same failure to close door at start of consultation </p> <p>I Signed the consent and indemnity to the procedure and as per the previous occasion I crossed out the paragraph giving Dischem blanket right to use whatever information in whatever way they see fit</p> <p>This time she refused to give the injection </p> <p>When questioned as to why last time when I crossed out the paragraph the procedure was not refused She threw her hands up in the air exclaiming " I really don't know"</p> <p>I took my complaint to the store manager who passed the buck to her supervisor who had no comment</p> <p>After she was unable or unwilling to find the requested telephone numbers of her superiors I left..</p> <p> </p> <p>Why is the procedure listed as a vaccination ? against what ? </p> <p> </p> <p>To Dischem Legal Department</p> <p> </p> <p>On the 15th December I was refused the administration of a Vitamin B Injection by a Michelle Labuschagne at your Randpark Ridge branch after signing the Indemnity and Procedure consent form and deleting the consent to release of information paragraph that gives Dischem blanket consent to share my personal information with whoever Dischem sees fit with out any specified limits </p> <p>I note that I have received sms’s from Dischem promoting specific products</p> <p>I was told that since I crossed out the release of information section she would not administer the Injection </p> <p>2 months previously I signed the form in the exact same way and received the treatment from the same person </p> <p>Please inform me on what legal basis the treatment was refused since it is my right to consent or not to my personal information being shared</p> <p>Also why are these 2 separate issues on one form ?</p> <p>And why is the procedure written as an administration of a vaccine ?</p> <p>Another matter is Dischem not in contravention of Schedule 3 procedures by not requiring a Doctors script</p> <p>I note that there is no prior notification any shape or form that treatment will be refused if the consent to general sharing of information is not agreed to</p> <p>PR Evans</p>
