Nurse appears to have dementia

Sister Claire G***** administered vaccine into my buttocks. As a registered nurse, she should have known that vaccine is ineffective if administered there.

When I found out and complained, she was rude and did not take accountability for her actions. She didn’t remember me, nor administering the injection in the buttocks. Further she neglected to record the dose and route correctly in the vaccination card. If the nurse is struggling to remember things like how to perform her duties, she should be dismissed.
