Duplicate I'D number on dis-chem system

Dis-chem has managed to link managed to link my ID and medical aid details to another person. I now have a duplicate account and my contact details are also linked to her profile. Despite numerous queries at the dispensary and multiple attempts to have this resolved I am still not any closer to a resolution. As you can imagine not only does this person now have my personal details and access to my medication, she also has access to my medical aid. All this and dis-chem is still unable to assist?
