Canselation of membership

Good day,

RE: FORMAL CANCELLATION OF MEMBERSHIP AND STOPPING OF DEBIT ORDER

I, Maartin van Staden, hereby formally request the immediate cancellation and termination of my Dial-a-Guard membership/service.

I further instruct that no further debit orders or payments may be processed against my bank account in respect of this membership after the applicable cancellation date.

Please provide me with written confirmation that:

1. My membership has been cancelled and terminated. 2. The cancellation has been recorded on your system. 3. The debit order has been cancelled/stopped. 4. No further amounts will be deducted from my bank account. 5. If there is any outstanding amount that you believe remains payable, please provide me with a full written statement and explanation before attempting any further debit.

Please treat this letter as my formal notice of cancellation and process the request as soon as possible.

I request written confirmation of the cancellation for my records.
