Delaying Pension Payout

I resigned as educator in December 2014. I will now be completing documentation to receive my pension money for the 5th time. My latest attempt was rejected because the Commissioner of Oaths stamp was in BLUE ink. Another question is why do I have 3 different pension numbers? I have always worked for the Gauteng Department of Education. I don't know what to do any more. I resigned because of depression and anxiety attacks meaning I am unable to work.
