Debt Review Listed in My Name Without My Knowledge and No Response to Dispute

I discovered that a debt review application was submitted under my ID number (***) through Debt Solutions Co on 10 March 2025.

I dispute ever applying for debt review. I have repeatedly requested copies of the alleged application, signed consent forms, call recordings, and other supporting documents.

Despite numerous emails, I have received no response. I have also experienced difficulty contacting the company through the contact details provided.

The debt review listing has negatively affected my credit profile and ability to obtain credit.

I am requesting that Debt Solutions Co provide:

The alleged debt review application. Signed consent documents. Proof of identity verification. Copies of all communications re**** upon as proof of consent.

To date, these requests have not been answered.

I am seeking a full investigation into how this debt review application was submitted and appropriate corrective action if it was done without my authorization.
