Cellsure has not sent me claim forms

My cellphone is insured with Cellsure and the screen cracked when I had slipped. I logged a call over a week ago and they still haven't sent me claim forms I called for the second time on Monday 2 February and they assured me that they would send claim forms but haven't yet.<br>
