Victim of ***** but listed as a *****ster

In October 2019, someone opened a *****ulent Cellc using my details. I sent an affidavit to *** stating that I did not open a Cellc account and the debit orders going from my FNB account should cease with immediate effect. The debit orders were ceased, but to my shock my name on the SAFPS. I think it was clear that I was a victim of *****, I would not have sent that affidavit if I was a *****ster. Could you please forward me all the documentation that was used to open that cellc account? I would like to see the signature and Id used?
