High charges for data and calls

I have been subjected to charges of R0.99 Per MB when using data outside of a data bundle. I would like to understand how this price is determined and deemed fair to clients this fee is ridiculous. To charge me R39 for updating apps, amounting less than 40MB when I am a long standing client who has repeatedly had to accept poor service and poor quality of connection on calls and data coverage, call and data connections being dropped and poor understanding by your call centre staff. Another problem is that to make a local call, within south Africa is more expensive than making a call of the same length to a land line or a cellphone number in America. Why are the connection fees so high? I would like to understand why we are being charged such high fees?
