Cell C Debit order MESS

Again debit order did not go off my account beginning of January, Cell C mistake and again they reported this to the credit bureau. It now shows I have not paid Cell C and my account is in arrears. How can this happen two times out of twelve months and the rest of the time the debit order goes off flawlessly?
