Cannot get a sim swap done

I’ve been trying to get a sim swap done and I cannot without 5 dialed numbers I told them I don’t have the phone anymore so I don’t have the 5 numbers , I’ve tried more than 3 times even have a case number from the police station to show that I lost the phone with the numbers
