Do you have my personal information and an unauthorized bank card?

This courier company somehow obtained my personal information from my bank. I never agreed to nor signed up for a system whereby a courier company would have any reason to interfere in my personal business with my bank. I indicated this much to both my bank and to this courier company. Yet I now have reason to believe that this courier company is in possession of both my personal information, and a bank card that I did not authorize to be created let alone passed around like a joint at a Phis concert.
