Centriq life insurance

I made a claim with the Capitec bank funeral cover but it was not approved because it was my cousin they say it because we dont share the same surname but when i made the cover i was not told that and in the capitec bank funeral cover documents it state that i can cover my cousin so am really confused ....before i take this matter further can somebidy make me understand or explain clear whats going on... Thank You
