contract i have no knowledge off

kindly assist, i have just discovered that i happen to have a contract with Blue Label connect, not sure how that happened and which forms did i sign to get that contract. now the problem is that i have lost my sim card and i cant do a sim swap with vodacom, they keep on referring me to Blue connect which takes me from pillar to post and not assisting at all.

please help
