Avon Justine has put me on credit bureau

Hello Peter <br> <br> My name is Mr MG Ngubo, the problem i have is that Avon Justin has put me on credit bureau without me opening an account or taking anything from them. I have reported this on the year 2014 my ref TU*** and the consulted who was helping me is Alphius Shabalala and when i last spoke to him he told me that this matter was resolved however i have visited the bank today and i have been told that i am still on credit bureau with Avon Justine. <br> Hello Peter please help me remove this from my name as it was *****.
