Andemnity forms Attridgeville Branch

I have been looking for andemnity forms. I went to attridgeville branch and I was told that they are not available they will contact other branches and once they get they will inform me. I called and still the same answer. Today I was in pretoria cbd and decided to go to the nearest avbob and ask for an andemnity form and for security reasons I was asked that I need to produce an ID card which I forgot to bring which I totally understand that I cannot breach security. My issue is why is there no andemnity forms in the attridgeville branch. When are they going to receive the andemnity forms. What is the point of having an attridgeville branch if I can't get any services from it.
