Still debited for someone elses account

I am currently being held liable for a contract that I did not take out or extend or provided any consent for.<br> <br> I have sent numerous emails and was sent the contract extension that wasn't taken out by me and I have provided proof that money is being debited off my account. Another debit order went through again today and this is simply unacceptable.
