wrong billing

<p>I took bloods November 2015 through my Dr, form was filled correctly with right details and correct medical aid number. Ampath decided to claim the bloods with wrong surname and wrong medical aid number. The accounts department called me, I told them to reclaim the bloods with correct details. The next thing I was handed over to the lawyers. With their own fault and ended up paying extra charges. I want Ampath to send the correct invoice with correct details . I am tired to be taken for a ride, because of not doing things properly.</p> <p>.</p>
