Lost sample

I did an papsmear exam at dischem clinic in strubensvalley on the 29 of april and after two weeks of not getting the results. I had to call and dischem told me that they submited my sample but you guys( ampath) lost it... imagine now i have to do another papsmear..

Pap smears are uncomfortable and invasive, and having to repeat this due to an administrative or lab error is extremely disappointing. I trust that proper procedures are in place to handle and process such important tests, and I hope this is being taken seriously to prevent it from happening to other patients in the future.

I would appreciate a clear explanation of how this occurred and what steps are being taken.
