Claim submitted with wrong dependant code

Ampath Pathologists handed my account to 3rd party for collection how ever when I call Discovery to find out why the account was not paid for R1127.80 Discovery said Ampath used wrong dependent code for the claim meaning Ampath claimed for me and the patient was my son.So my problem is,Ampath doesnt want to provide me with copy of claim they submitted to discovery or amend the claim and resubmit how ever discovery is willing to pay that amount if they get correct claim documents.I have called Ampath many times they said they submitted correct claim.
