Incorrect termination date on UI-19

I requested to be assisted with UI-19 on December 18, 2019, and I only received it on 7 January 2020 with an incorrect termination date. the termination date was supposed to be 18 October 2017 but the one sent to me was 01 March 2019. I called the HR several times to correct the termination date but was always promised I will receive the correct documents the same day I called for which it never happened. Yesterday (4/02/2020) i spoke with the HR who promised that she is done with the documents and will send it in 15 minutes and nothing happened. I sent a reminder mail but it was not responded to. I'm still waiting for the correct documents and no one is responding to my mails.
