Repay me and stop taking my money

I requested their services and filled out forms.they had to come program something in the fone but never came.they also had to come put their sign board it never happened.we agreed on 99 a month.but they take 112 a month but these no services.i tried calling for cancellation to no avail. Some months they take double debit from my account.99&112.i stopped it at the Bank but they keep changing reference and take my hard earned money for nothing. Adt please stop.?
