Black listing the incorrect person

We had a ADT account more than a year ago. ADT came and removed their alarm system. The account was in my wifes name to my surprise my ID number has been blacklisted. I need an explanation why my wife never received an account of her outstanding balance. And I need a proper explanation why my name was blacklisted.
