Woolworths Insurance Claims

I have send through a claim for dread disease on the 23/03/2018. after a few days I phoned in to follow up regarding the claim, only to get told that they did not receive anything! then I send the forms again ! yesterday after sending the forms 6 times already I was yet told again .. No forms were received ! this is going on for a month now !
