Poor customer service

I app**** for a store card and filled out the application form at Morning Glen WW. The information captured has the incorrect surname and now I am told it would be declined or I would have to reapply because of the incorrect surname. I am not sure how I ended up with the surname captured onto the application and my WRE card as well.
