Credit card that was activated without my concern

I have been complaining about the credit card that I got emails about since 2015 and when I call or email woolworth's the take me to pillar to post. They send me the dispute form to complete and I didn't complete it because their questions seems as if I gave the card to someone else to use or they use it without my concern all I want is that credit card to be cancelled I only receive an sms which says I should go n collect the credit card with my od and proof of residence and I didn't go there as I know that I don't want that credit card but to my suprise I get emails that shows me that I owe them. If they can show me the I'd and that proof of res that activated that credit card
