What is going at at Willoughby's?

I've been at patron at this restaurant for many years, for its superb food and excellent service, until recently. I've been singing Willoughby's praises to my overseas friends and was left rather embarrassed when on two occasions last week encountered shocking service. So unlike this place. Do they have lots of new and insufficiently trained staff there now ? The first incident occurred early on Monday evening when it was not particularly busy, 3 out of 4 people in our group all received their food at different times (not together), although we had placed our order all at the same time. Only after the waitress served the 3rd person's food, she realized that she had forgotten to place the food order for the 4th person. The 4th person then received her food only after the other 3 had finished eating. On Friday night we were a party of 5. When the drinks arrived, one person who had ordered a bottle of water, received just the bottle, no glass. The bottle of wine shared by 2 other people, was simply placed on our table, still sealed and unopened. We had to call back the waiter to do what he is supposed to do. What on earth is going on at Willoughbys??
