Unable to get Virgin Active to cancel membership

I have been trying to cancel my husband membership since last year. I have handed in his cancelation letter twice at VA Middelburg and each month I reverse the debit order. They still also have not refunded the 2 x debit orders after canceling that they took before I realized they were still deducting his membership fees.
