***** committed by a virgin active staff member

A staff member at virgin active forged my signature and signed me up on a 2 year contract, I have phone the head office, logged a complaint at the gym, emailed my consultant but I’m not getting any response, can someone advise on how do I go about reporting a ***** case committed.
