Dispute form not recieved but sent

I need assistance with a dispute logged on the 1/02/23.

I recieved an sms to say the dispute form was not signed. I checked and all the documents were sent signed. I resent them anyway. Called a few days later to follow up, I was advised that on the documents the dispute form was not attached even though it was. I resent but am still not getting any assistance. Kindly assist.
