UNAUTHORIZED DEBIT

The agent approved the debit on my phone without my knowledge. I specifically told her that I will make a decision on whether I want this policy or not after I have received the phone call (which she told me I was going to receive) and she said it's okay it won't active until I give the go-ahead, two days later I receive a call and I was told that my policy has been approved. How? When I check my banking app it shows that a debit was approved, I didn't approve any debit. The act was *****ulent, and I wouldn't trust any of the Unlimited agent.
