Disappointed and misled

I'm disappointed once again..... How can I submit my Gap cover claim on the 9th of April and was never told that I have used an old claim form , no one raises this up until I was the one who had to phone them.... I even received an email saying that I submitted everything and today they are telling me about the old claim form and I have to fill in the new claim form.
