Concerns About Record Accuracy

“I consulted with Dr. Kanthan at Durban Pain Clinic. During the consultation, another doctor, Hudson, was in the room working on other cases. However, the medical notes in my file for that visit appear to be in his handwriting, not Dr. Kanthan’s. The form also was not signed by her. This makes me worried about the accuracy of my medical record, since she was the one consulting me. I have requested clarity from the clinic on this matter.”
