Lost File

20 November 2017 - Came to consult and asked for a maternity leave letter which I need to submit to my employer. I registered at the reception and left my green card there as always, which I was supposed to get at the consultation room with my clinic file. I went to the consultation room my green card and clinic file were not there. I'm told my file was taken out. I need my file and even worse I was not helped. I'm told they don't write maternity leave letters I need to consult a private doctor. One guy offered to create a new green card for me since I said I'm going to work and I'm running late. I refused because I'm afraid once I leave the clinic they wont look for my misplaced file. It's been almost 45 minutes now since I'm waiting for my green card. I should be long gone. I need this matter dealt with and I would appreciate it if they could give me my file back. I'm not happy with the service, it is their duty to keep files safe.
