Missing accident report form in Thembisa hospital

I was involved in a accident on new year's eve (31 /12/21), from the scene I went with an ambulance to Thembisa hospital where I was treated and discharged. The doctor gave me a date to come back on the 17 January 2022. I went there and the doctor told me that my file does not have any medical record and she don't know I must be attended on what then she asked me what happened and where was I hurt ,they just took me for x ray ,give me meds and I was discharged.

On Tuesday I called the hospital to ask why my file was not having any medical report (the accident report) they gave me to contact that doesn't even go through to inquire. I want to know why is my accident report not in my file. Please help
