cancellation of debt review

I app**** for debt counselling with summit but I didnt sign the consent forms to start with the process as they could not negotiate with other creditors. Going through with the process would not help me because I will still be liable to pay other creditors and that would not give me any financial relief. I went to a financial institution to get a loan and was told that I am under debt review with summit whereas I am not. I called them and they told me that I have signed some forms and they cannot help me and if I want to resolve this issue I must go to court myself and have it sorted out maybe I will succeed. I asked them what their plan is because they are the ones who labelled me and now I cannot even get a loan because of that record and all they said was they will call me and they never did. All I wanted was proof that i am not under debt review with them or anyone for that matte. Please help me resolve this issue
