Standard bank opening *****ulent funeral policies

The is a policy that is open on my behalf without my knowledge and when i am trying to cancel I have been transferred to the cancellation department but no body answers phone, I would appreciate a formal response explaining how these policies were opened without proper verification and what Standard Bank intends to do to address this issue.
