Standard bank credit card insurance rejected

Good day standard bank credit card insurance.

in 2020 July during covid-19 had a heart attack was taken to south rand hospital then taken to jhb general hospital where spent almost 6months there - went for open heart surgery in December 2020 where they pealed my right side of heart was like cement was 129kg off water normally I'm 65/70kg - 300mg of iv for taking out water in my body unable to walk 10 steps. But your insurance inspector came set Forms saying fill them out done then out emailed him after that says fill this forms same info was on both forms jhb had the fire at that time Taken me longer to fill them out an was rejected for some reason. They was no covid-19 help an had a heart few attacks. Have photos as proof what happened an few documents which was sent.
