Months for a refund and cancelation of policy

It takes months to cancel my home owners insurance and life cover, still waiting for a refund 3 months later and when I want to sell my property now there is a life insurance amount on the cancelation sertificate. Please provide the details for the ombudsman where I can log a complaint as I am tired of trying to sort it out with standard bank employees
