Debit order deducted AGAIN after policy holder has passed

After fighting for a month (without any success), Standard Bank Insurance has AGAIN deducted a debit order off my account for a funeral policy where the policy holder has passed away and the policy closed. HOW DO YOU DO THAT AGAIN?!?!?!?

I have now lodged an official complaint with the Short Term Insurance Ombudsman. Hopefully I will now get some sort of response.
